| 目的 探讨有限切开结合空心拉力螺钉内固定治疗Ruedi-AllgowerⅡ型pilon骨折的疗效.方法 自2008年8月至2012年12月收治并随访患者23例,男17例,女6例;年龄13~58岁;闭合性骨折19例,开放性骨折4例.骨折按Ruedi-Allgower分型均为Ⅱ型;伤后至接受手术时间3h~22d,平均11d.手术采用内踝前、后方弧形切口有限切开复位后,空心拉力螺钉呈倒三角形固定.结果 本组23例患者术后伤口Ⅰ期愈合,2例出现伤口小面积皮肤坏死,经清创换药后愈合.随访时间6~24个月,平均15个月均获得骨性愈合,愈合时间13~25周,平均愈合时间18周.采用AOFAS评分标准评估,23例患者术后评分72~93分,平均82.5分;其中优18例,良3例,可2例,优良率91.3%.结论 有限切开结合空心拉力螺钉内固定治疗Ruedi-AllgowerⅡ型pilon骨折具有创伤小,操作简便,术后恢复快,并发症少等优势,是一种值得借鉴的术式. |

| Objective To investigate application of minimally invasive approach combined with internal fixation in the treatment of tibial pilonfractures. Methods Totally 23 patients suffered from Ruedi - Allgowertype fl pilonfractures, and 19 of them wereclosed fractures, the rest were open fractures. The fractures were reset on the basis of minimally invasive notch and internalfixed with hollowscrews. There were 17 male and 5 female in the 23patients, aged from 13 to 58 years old, got hurt and treatment during January 2008 and September 2012. Results All the 23 patients acquired phase I cicatrization after sugary, got bony union within 15 months on average （They were followed - up for 6 to 24 months）. The average cicatrization time was 18 weeks （ ranged from 13 to 25 weeks）. The score of AOFAS ranged from 72 to 93 and 82.5 on average. The excellent rate was 71%. Conclusions Minimally invasive approach combined with internal fixation in the treatment of tibia1 pilonfractures for it has the advantages such as small trauma, simple operation, rapid postoperative recovery, fewer complications. Worthy of expanding. |
